What it is

One identity, every care moment.

An HPN is a unique, permanent identifier issued to every person in Nigeria — patients and clinicians alike. It is different from a hospital file number, which only works at one facility. Your HPN works everywhere PHB-connected providers can see a record.

You receive it once at verified onboarding. It cannot be revoked, reassigned or transferred. Even doctors get one — because a clinician is also a patient.

Universal across Nigeria
Every PHB-connected hospital, pharmacy and lab reads the same record by your HPN — not just the one that issued it.
Clinicians get one too
Doctors, nurses and pharmacists each have an HPN. A clinician is also a patient — one identity covers both roles.
Works without a smartphone
Access your HPN via USSD on any network (*894#), IVR on 0700 PHB CARE, or simply show up at a PHB-connected front desk.
Not the same as your hospital number
A hospital file number is local. Your HPN is national. It travels with you, not with the building.

Showing your HPN does not open your record.

Giving a provider your HPN allows them to look you up in the network. It does not automatically share your clinical data — they still need your real-time consent to read your record.

Every access — who saw what and when — is logged in an append-only audit trail. You can review it at any time from any channel.
